What Care Level Calculator does
Since 2017 Germany has had five care levels. Which one applies is not decided by a diagnosis or a number of minutes but by a points system: the assessment instrument under § 15 SGB XI. The Medical Service looks at six areas of life, awards points for each question and converts them into weighted points with a fixed table. From 12.5 of 100 points there is care level 1, from 27 care level 2, from 47.5 care level 3, from 70 care level 4 and from 90 care level 5. The calculator reproduces exactly this system, with the questions and point values from Annex 1 and the conversion from Annex 2.
The six areas count differently. Self-care weighs most at 40 percent: washing, dressing, eating, drinking, toilet. Medical care, meaning tablets, injections, dressings and doctor visits with company, counts 20 percent. Moving counts 10 percent, daily life and contacts 15 percent. Understanding and talking on the one hand and behaviour and mood on the other share 15 percent, and only the higher value counts. This matters especially with dementia: someone who still seems physically fit can still reach a high care level through these two areas.
The calculator is built for people who do not sit at a computer every day. Each area is its own step, each question a card with big answer fields in plain words: "Manages alone", "Needs a little help", "Needs a lot of help", "Cannot do it". Below it, in small print, is the name of the question in the official report, so you recognise it in the decision later. Your answers stay in your browser only and are deleted when you sign out.
Children have their own rules, and the calculator knows them. From 19 months only what a child needs on top of a healthy child of the same age counts. For babies and toddlers up to 18 months the Medical Service only rates behaviour, medical care and major feeding problems, and under § 15(7) SGB XI one grade higher applies automatically. And anyone who can no longer use both arms and both legs gets care level 5 under the so-called special needs constellation, even with fewer points.
After the result comes the practical part. The calculator explains how to apply to the care insurer (a phone call is enough), that money flows from the month of application and that under § 18c SGB XI the insurer must decide within 25 working days. Enter the date and the calculator shows the last day, with the public holidays of your federal state. One click puts the deadline into the deadline tracker. It also describes how the Medical Service visit works, what to have ready and why a care diary helps so much.
One last important point: the calculator gives an estimate, not a decision. The care insurer sets the care level, based on the Medical Service report, and in individual cases the assessor may see things differently. For 2027 the federal government plans higher thresholds for care levels 1 to 3 with the care reorganisation act; the draft is not law yet. The calculator uses the law in force in 2026 and shows the planned difference as a note. What the care level brings in money you then work out in the care allowance calculator.
Features
The real assessment questions
Every question from Annex 1 to § 15 SGB XI in plain words, with the official point values and the conversion from Annex 2.
Made for every pair of hands
One area per step, big answer cards, a progress bar. Works on a phone and can be filled in calmly over several days.
Points per area as bars
You see where the points come from and how many are missing for the next care level. That helps to check the report later.
Children and special cases
Comparison with same-age children from 19 months, one grade higher up to 18 months, and care level 5 when both arms and legs can no longer be used.
The insurer deadline in view
25 working days from the application under § 18c SGB XI, with public holidays per federal state and a button for the deadline tracker.
On to the care allowance
One click opens the care allowance calculator, which shows the benefits that go with the care level.
How it works
- 1
Choose who it is for
Adult, child from 19 months or baby up to 18 months. The calculator then adapts the questions and rules.
- 2
Answer area by area
Think of an ordinary day, not the best one. Aids count. You can go back any time or continue later.
- 3
Read the result
The likely care level, the points per area and what it means in benefits. Print or save as PDF with one click.
- 4
Apply and save the deadline
Call the care insurer, enter the date and let the deadline tracker remind you when the decision is due.
Who needs this
Frequently asked questions
How accurate is the care level calculator?
It uses the same questions, points and tables as the law. The result is still only an estimate: the assessor sees the person, reads medical reports and may judge an individual case differently. The care insurer sets the care level. The calculator replaces neither the assessment nor care advice.
Where do I apply for a care level?
With the care insurer. It sits with the health insurer of the person who needs care. An informal call or a short letter is enough, the form follows. Benefits are paid from the month of application (§ 33 SGB XI). Privately insured people contact their private care insurer.
How long may the care insurer take?
At most 25 working days from receipt of the application (§ 18c(1) SGB XI). If it is later through its own fault, it must pay 70 euros for every started week (§ 18c(5)). In hospital, rehab or palliative care shorter deadlines apply.
What is a care diary and do I need one?
A simple list in which, for one to two weeks, you write down every day what you help with: getting up at night, tablets, washing, calming, trips to the doctor. At the Medical Service visit many people pull themselves together and seem fitter than they are. The diary shows real everyday life and makes it easier to describe everything.
Will the care levels change in 2027?
It is planned. On 30 September 2026 the federal government adopted the draft care reorganisation act. From 1 January 2027 it plans higher thresholds: care level 1 from 15, 2 from 30, 3 from 50 points; care levels 4 and 5 stay at 70 and 90. The conversion of the points is also to change. Under the draft, the law on the day the application reaches the care insurer applies. It is not law yet; the calculator uses the 2026 values and shows the planned change as a note.
What can I do if the decision does not fit?
Object within one month of the decision (§ 84 SGG). Ask for the report, compare it question by question with your estimate and note where something is missing. Free help is available from care advice, local care support centres and social associations.
Are my answers stored or sent?
The calculation runs entirely in your browser. So that you can continue later, the answers stay stored in this browser for your account and are deleted when you sign out or choose "Start over". Only if you add the deadline to the deadline tracker does a title with the date go to your account, no health details.
